We also understand choosing a care provider for yourself or a loved one can be a daunting task. We aim to make the whole process a simple and enjoyable one. At Primary Care we work closely with you and your family or carers to ensure together we design a care package to meet your specific individual needs, to provide exactly what you need, in the way you wish.
So whether you are looking for just a few hours support a week or 24 hour care we are able to offer the help you need to meet all of your requirements. We offer a range of care and support services to enable you to remain living in your own home which we can arrange, coordinate and deliver in a fast, hassle-free way.
Some of the services available from Primary Care include:
Home Help – to carry out tasks on your behalf such as, shopping, cooking, cleaning, personal care, gardening, dog walking and handyman services. These may be just some of the things that you are no longer be able to fully manage yourself.
Home Support – to support you to remain as independent as you are able, including help with personal care, medication, meal preparation, housework, mobility and heath related tasks.
Community Care – to help you get out and about more within your community, use community facilities, attend appointments, meet new people and discover new interests.
We can also support you with 24 hour care, night time sleep in support, and night time awake support.
If you are an older person living at home in need of care and support or you are a carer who requires respite support, you may be entitled to an assessment from your local social services department to see if you may be able to receive funding to pay towards your care and support.
